Accommodations Forms
Forms for requesting special accommodations are listed below. The process of requesting accommodations begins and ends with the local test center. Examiners will provide guidance and assistance in completing the form, and you must return the form to the test center before it is reviewed. A form sent to the GED administrator by anyone other than a GED examiner will be returned to the sender.
